I remember someone referencing this not too long ago. Can't remember the context, but thought I'd put this here since I found it for sale on Dillon. Has a really small bell option: 6"!
This is a custom made Conn trombone model 62H for Gardell Simons former principal trombone of the Sousa band, Philadelphia Orchrestra, Detroit Symphony, etc...The instrument is in good condition, but does have lacquer wear throughout. With original case and has detachable 6" and 7.5" bell. Bore size is .484

That was probably me. This same horn is shown at the conn loyalist using the original pictures that Dillon music had. I wonder what happened to the counterweight, though it may not have been original.
Now, part of my brain swears that there was another Gardell Simons trombone that was listed at the same time years ago, and that had an F attachment (pancake wrap) but I can't remember if that also had two bells.

Yes, very interesting horn. It must have been there at Dillons for many years now. It was there when I started to buy horns 2014-15. I remember it had the same model number as the bass trombone, was way too expensive back then, and probably is also today.
